Forms/Rental Information on Track and Cabin
Minor Release Form- Either (1); Must be signed and notarized by legal parent/guardian, must be printed in color. Signatures and Notary seal must be originals, (not a copy). Or (2): Parents/Legal Guardians who have not notarized their documents must have signatures verified and witnessed by a Dream Promotions inc. (Field of Dreams) representative at sign up.
Parents/Legal Guardians who have placed minors in the care of another adult must provide a separate notarized letter authorizing medical authority in the event of accident or illness. Such permission should also indicate known allergies and the dosage levels of any prescribed medicines. Do not forget to provide your responsible adult with medicines, inhalants or epipens if necessary.

Riders Accident Insurance for AMA Sanctioned Events- All riders should consider the possibility that their health insurance provider may have exclusions to motor sports activities and purchase an appropriate supplemental policy where necessary.
